PROGRAM NAME Woodside Transition House
PROGRAM OVERVIEW The Woodside Transition House is predominantly a step-down program for youth transitioning out of the Woodside Juvenile Rehabilitation Center.  The primary goal of the Woodside Transition House is to promote and support successful transitions for the youth in our care, back into their communities.
WHO WE SERVE The Woodside Transition House serves youth, ages 16-22, who are transitioning out of the Woodside Juvenile Rehabilitation Center.
WHERE WE PROVIDE SERVICES Most of our services are provided at the Woodside Transition House in Burlington.  However, our services are individualized and, as needed, mobile and provided throughout surrounding communities.
WHAT SERVICES WE PROVIDE The Woodside Transition House provides transitional support services for youth through a 4-bed residential treatment program in Burlington.  Transitional services include:

  • therapeutic milieu treatment
  • case management
  • vocational supports
  • transportation
  • transition planning.
HOW SERVICES ARE FUNDED Woodside Transition House services are funded directly through the Vermont Agency of Human Services (AHS).  There is no fee to parents or youth for participating in the services provided through our program.
HOW TO MAKE REFERRALS All youth receiving treatment at the Woodside Transition House are referred by the Woodside Juvenile Rehabilitation Facility or their DCF social worker.For more information about the Woodside Transition House, contact Chris Smith at christophers@howardcenter.org or (802) 488-6702.
